The "Disclosure Report for 868 Meadow Road" is taken from Section 35 of the Illinois State Legislature's (765 ILCS 77/) "Residential Real Property Disclosure Act."

(765 ILCS 77/20)
Sec. 20. Disclosure report requirements. A seller of residential real property shall complete all items in the disclosure report described in Section 35. The seller shall deliver to the prospective buyer the written disclosure report required by this Act before the signing of a contract.

The "Disclosure of Information of Lead-Base Paint" comes from the following legislation passed in 1996:

"The Disclosure of Information on Radon" is from both Federal government work and the Illinois State General Assembly.
The EPA and the radon industry led ANSI/AARST voluntary consensus-based standards process which has been ongoing since 2012. There is currently no Federal legislation, guidelines or requirements in relation to Radon Disclosures.

Illinois State Assembly passed the Illinois Radon Awareness Act on January 1, 2008 and requires all sellers or property owners in a residential property transaction in Illinois to disclose any information on the history of radon in the home.
The revised, the "Multi-Board Residential real-Estate Contract 7.0" is the most widely-used contract in Illinois for residential real estate transactions.
